ABSTRACT

Background. Diverticular disease was previously thought to be non-existent in the black African population. Studies over the past four decades, however, have shown a steady increase in the prevalence of the disease.Objective. To report on the profile and current prevalence of diverticular disease in the black South African (SA) population at Dr George Mukhari Academic Hospital, Pretoria, SA.Methods. A retrospective descriptive study was performed in black SA patients who were diagnosed with diverticular disease by colonoscopy between 1 January and 31 December 2015.Results. Of 348 patients who had undergone colonoscopies and who were eligible for inclusion in this study, 47 were diagnosed with diverticular disease ­ a prevalence of 13.50% (95% confidence interval 10.30 - 17.50). The greatest number of patients diagnosed were in their 7th and 8th decades, with an age range of 46 - 86 (mean 67) years. There was a female predominance of 57.45%. Lower gastrointestinal bleeding was the most common (65.96%) indication for colonoscopy. The left colon was most commonly involved (72.34%), followed by the right colon (55.31%). A substantial number of patients had pancolonic involvement (27.65%).Conclusion. This retrospective study suggests that there has been a considerable increase in the prevalence of diverticular disease among black South Africans, possibly owing to changes in dietary habits and socioeconomic status

ABSTRACT

Colonoscopy is a safe and effective means of visual inspection of the large bowel from the distal rectum to the caecum. It may be carried out for diagnostic and or therapeutic reasons. There is a paucity of data on this procedure in Nigeria. We; therefore; determined the indications; findings; and diagnostic yield in Nigerians at colonoscopy. Materials and Methods: This was a hospital-based cross-sectional study carried out at the Endoscopy unit of Crescent hospital; Ilorin from January 2010 to May; 2012. The endoscopy register was reviewed; and the biodata; indications and colonoscopic findings were recorded on a pro forma. Results: A total of 103 patients had colonoscopy. Seventy (68.0) were males while 33 (32.0) were females. The indications for colonoscopy were rectal bleeding 41 (39.8); suspected colon cancer 32 (31.1); chronic constipation and chronic diarrhoea nine each (8.7); abdominal/anal pain five (4.9); suspected anorectal cancer and enterocutaneous fistula two each (1.9); faecal incontinence; occult gastrointestinal bleeding; post-colostomy for Hirschsprung disease one each (1.0). Endoscopic findings were normal findings 21 (20.4); diverticulosis 17 (16.5); polyps 16 (15.5); haemorrhoids 16 (15.5); anorectal cancer 13 (12.6); angiodysplasia 12 (11.7); colon cancer eight (7.8); colitis 7 (6.8); anorectal ulcer 4 (3.9); anal warts two (1.9); anal fissure; caecal tumour; faecal impaction and proctitis one each (1.0). The diagnostic yield was 79.6. Conclusions: The commonest indication for colonoscopy was rectal bleeding; while the most frequent pathology was diverticulosis. The diagnostic yield was high

ABSTRACT

Objectif : Preciser les aspects cliniques; para-cliniques et therapeutiques du diverticule de l'uretre feminin dans le services d'Urologie du CHU du Point G . Patients et methodes : Il s'agit d'une etude retrospective portant sur dix patientes ayant ete hospitalisees avec le diagnostic de diverticule uretral dans le service d'urologie de l'hopital du Point G. L'examen clinique de nos patientes; les examens para cliniques biologiques et d'imagerie medicale ont permis de poser l'indication operatoire. Resultats : La majorite de nos patientes etaient secretaire de bureau. L'age moyen etait de 32 ans avec des extremes de 28; 65 ans. Dans les antecedents gyneco obstetricaux 80ont eu des accouchements normaux et 20etaient des nullipares. Toutes les patientes sont sans antecedents chirurgicaux. L'infection urinaire a repetition se retrouvait chez toutes nos patientes malgre la prise des antibiotiques. La symptomatologie du bas appareil urinaire a ete exprimee par nos patientes (pollakiurie; brulures mictionnelles; dysurie).et la dyspareunie par certaines. Le toucher vaginal retrouve une tumefaction sous uretral chez toutes nos patientes. L'echographie endo-vavinale a montre la masse sous uretrale a contenu liquidien evocatrice d'un diverticule sous uretral ou d'un kyste sous uretral dans tous les cas. La prise en charge de toutes nos patientes a ete chirurgicale (position gynecologique; meme technique operatoire : la Diverticulectomie transvaginale. L'histologie n'a signale aucun cas de processus neoplasique. Les suites immediates post operatoires ont ete simples. Aucune patiente n'a signale une symptomatologie urinaire apres six mois. Conclusion : le diverticule de l'uretre de la femme est une affection rare; souvent meconnue; son traitement est chirurgical
